Yes — medical dermatology visits and treatment are qualified medical expenses you can pay with HSA or FSA funds, but purely cosmetic dermatology procedures are excluded.

What flips the answer

  • Covered when

    The visit or procedure treats a medical skin condition, screens for disease, or is reconstructive after injury or disfiguring disease.

  • Not covered when

    The service is purely cosmetic — Botox for wrinkles, aesthetic laser or peels — with no medical condition being treated.

The IRS counts fees paid to physicians, including dermatologists, for diagnosing and treating medical conditions as qualified medical expenses. A visit to have a suspicious mole checked, treatment for acne or eczema, a skin-cancer screening, or a biopsy can all be paid from an HSA or FSA — including the copay your insurance leaves you.

Prescriptions that come out of a dermatology visit are eligible too. Prescribed medications for a skin condition are qualified, as are many over-the-counter treatments, which the CARES Act made HSA- and FSA-eligible without a prescription in 2020.

Where it stops is cosmetic care. Procedures that only improve appearance — Botox for wrinkles, cosmetic laser treatments, chemical peels for aesthetics — are excluded as cosmetic surgery unless they treat a deformity from a congenital abnormality, injury, or disfiguring disease. The same dermatologist can provide both eligible and ineligible services in one visit, so it comes down to the medical purpose.
