There's a narrower path when massage is delivered as part of medically necessary care. Some plans cover manual therapy or therapeutic massage when a physical therapist or other covered provider performs it as part of a treatment plan for a diagnosed condition, ordered by a provider and billed under a covered code.
Even where that path exists, plans usually require a prescription or referral, a covered diagnosis, an in-network provider, and adherence to visit limits. A licensed massage therapist working outside a covered treatment plan generally can't bill your medical insurance.
